Eisai Presents Data on Benefits of Long-Term Administration of Dual-Acting Lecanemab at the 17th Clinical Trials for Alzheimer’s Disease (CTAD) Conference
TOKYO and CAMBRIDGE, Mass., Oct 31, 2024 - (JCN Newswire via SeaPRwire.com) - Eisai Co., Ltd. and Biogen Inc. announced today that the latest findings for lecanemab-irmb (U.S. brand name: LEQEMBI®), an anti-amyloid beta (Aβ) protofibril* antibody for the treatment of early Alzheimer’s disease (AD), were presented at the Clinical Trials for Alzheimer's Disease Conference (CTAD), held in Madrid, Spain, and virtually.Benefits of Continued Treatment with Lecanemab for People with Early ADIn July 2024 at the Alzheimer's Association International Conference (AAIC) 2024, results from the open-label long-term extension study (OLE) following the core study of the lecanemab Phase 3 Clarity AD study were presented, showing that the mean change from baseline in CDR-SB (global cognitive and functional scale) in the lecanemab treated group relative to the placebo group was -0.45 at 18 months, and at 36 months, this expanded to -0.95 compared to a prespecified natural history** cohort of AD. There was a 30% reduction in the relative risk of progressing to the next disease stage In addition, the tau PET substudy of the lecanemab Phase 3 Clarity AD clinical study showed that with three (3) years of continuous treatment with lecanemab, 59% of patients with no or low tau accumulation in the brain (no tau/low tau) at baseline showed improvement or no decline, and 51% showed improvement from baseline on the Clinical Dementia Rating-Sum of Boxes (CDR-SB) global cognitive and functional scale.1Clarity AD data presented at CTAD expand on these initial results to include additional measurements resulting from three (3) years of continuous lecanemab treatment in patients with low levels of amyloid accumulation in the brain at baseline (less than 60 Centiloids: low amyloid). These data show that 46% of patients improved or had no decline, and 33% showed improvement from baseline on the CDR-SB. On the ADAS-Cog14 measurement scale, 46% of patients showed improvement or no decline and 43% showed improvement. On the ADCS MCI-ADL, 51% of patients showed improvement or no decline and 48% showed improvement. These results – from no tau/low tau population and low amyloid populations – suggest that earlier initiation of lecanemab treatment may have a positive impact on disease progression of early AD patients and may provide continued benefits to patients with early AD over the long term.2No new safety findings were observed with continued lecanemab treatment over three (3) years. Most amyloid-related imaging abnormalities (ARIA) occurred in the first six (6) months of treatment. After the first six (6) months, ARIA rates were low and similar to ARIA rates on placebo during the placebo- controlled period. With regards to the incidence of ARIA by ApoEε4 status during the continuous treatment, the incidence was higher in ApoE4 homozygotes than in heterozygotes or non-carriers, but rates of new ARIA were decreased after the completion of the 18 months core study as treatment continued, regardless of ApoEε4 status.2Correlation between Protofibrils and Biomarkers for Neurodegenerative Disease in the AD Brain Dual-acting lecanemab is the only early AD treatment available to support neuronal function by clearing the highly toxic protofibrils that continue to cause neuronal injury and death even after plaques have been cleared from the brain. Protofibrils accumulate early in the AD brain and lead to nerve cell function loss, abnormal nerve processes, inflammation, and memory loss. In non-clinical studies, antibodies against protofibrils prevented protofibril-mediated neuronal dysfunction and memory loss.Accurately quantifying the amount of protofibrils in human cerebrospinal fluid (CSF) has been challenging due to their low concentration. As such, a new measurement method was developed by researchers at Eisai to accurately quantify protofibrils in CSF.Utilizing this new method of measurement, the amount of protofibrils in AD CSF correlated more strongly with neurodegenerative disease biomarkers (CSF total tau and neurogranin) than with CSF Aβ42, a biomarker associated with Aβ plaques accumulation, indicating that protofibrils are closely related to synaptic dysfunction. Furthermore, it was observed that protofibrils, unlike plaques, are diffusible. These results suggest that protofibrils induce synaptic dysfunction, playing an important role in neurodegeneration in AD brains.3Lecanemab Treatment for Early AD: Insights from Long-Term U.S. Clinical StudiesDr. Marwan Noel Sabbagh, Moreno Family Chair for Alzheimer's Research and Vice Chairman for Research and Professor, Department of Neurology, Barrow Neurological Institute, presented outcomes of an analysis of the use of lecanemab treatment between January 6, 2023, and July 30, 2024, based on payment claims data from the Komodo Research Database, a medical database in the U.S. In the U.S., lecanemab is used in accordance with the US FDA-approved indication, dosing, and monitoring guidelines. The analysis found that access to lecanemab treatment is expanding and highlighted opportunities to improve access in rural areas and educational outreach for underserved populations.4Dr. David Watson of the Alzheimer's Research and Treatment Center reported on patients who continued to receive lecanemab treatment following the Phase II Study 201 and Phase III Clarity AD study. A total of 136 patients participated in both studies at this center, and 66 patients chose to continue lecanemab therapy, with 13 patients receiving treatment for more than five (5) years and 40 patients receiving treatment for more than three (3) years. More than half of the patients (15/24) who continued treatment with lecanemab for more than three (3) years after the core phase remained in their initial stage of disease. Further, in a survey of 11 patients (or their caregivers) who received lecanemab treatment for more than five (5) years, all patients responded that they were “very satisfied” or “satisfied” with lecanemab treatment. In addition, between 45% and 73% of patients responded that lecanemab treatment made them feel more positive about their daily life, social activities, memory, etc. "frequently" or "very often."5No new long-term safety findings were observed in these multi-year studies.5Progress in the AHEAD 3-45 Study: Improving Screening Eligibility Using Blood Biomarkers and Completing Patient EnrollmentAHEAD 3-45 is a Phase 3 clinical study for individuals with preclinical AD, meaning they are clinically unimpaired but have intermediate or elevated levels of amyloid in their brains. In the study, blood tests, cognitive function tests (PACC-5***), amyloid PET, MRI, and tau PET were used for screening. Based on the amount of Aβ accumulation in the brain as determined by amyloid PET, subjects were assigned to two (2) trials with different dose settings: the A3 trial, for those with borderline Aβ levels in the brain, and the A45 trial, for those with positive Aβ levels in the brain.6Screening with blood biomarker tests was important to improve eligibility for amyloid PET testing in subjects without cognitive impairment. Using plasma Aβ42/40 ratio and p-tau217/tau217 ratio in theinitial screening reduced screening failure on amyloid PET from more than 70% to less than 30%. The primary endpoint was the global cognitive and functional scale, Clinical Dementia Rating Sum of Boxes (CDR-SB). In the Clarity AD clinical trial, treatment with lecanemab reduced clinical decline on CDR-SB by 27% at 18 months compared to placebo.9,10 The mean CDR- SB score at baseline was approximately 3.2 in both groups. The adjusted least-squares mean change from baseline at 18 months was 1.21 with lecanemab and 1.66 with placebo (difference, −0.45; 95% confidence interval [CI], −0.67 to −0.23; P<0.001). In addition, the secondary endpoint from the AD Cooperative Study-Activities of Daily Living Scale for Mild Cognitive Impairment (ADCS-MCI-ADL), which measures information provided by people caring for patients with AD, noted a statistically significant benefit of 37% compared to placebo. The adjusted mean change from baseline at 18 months in the ADCS-MCI-ADL score was −3.5 in the lecanemab group and −5.5 in the placebo group (difference, 2.0; 95% CI, 1.2 to 2.8; P<0.001). The ADCS MCI-ADL assesses the ability of patients to function independently, including being able to dress, feed themselves and participate in community activities. The most common adverse events (>10%) in the lecanemab group were infusion reactions, ARIA-H (combined cerebral microhemorrhages, cerebral macrohemorrhages, and superficial siderosis), ARIA-E (edema/effusion), headache, and fall.In July 2024 Eisai presented 36-month data from the Phase 3 Clarity AD Open-Label Extension Study demonstrating that LEQEMBI-treated patients continued to show benefit at 36 months of treatment. In the 18-month core study of Clarity AD, there was a statistically significant difference in global cognition and function as measured by CDR-SB between the LEQEMBI and placebo groups. The separation in CDR-SB between the group that continued to receive LEQEMBI (early start group) and the group who switched from placebo to LEQEMBI (delayed start group) was maintained during the 6-month OLE following the core study. This indicates that similar disease trajectory for both early and delayed start groups occurred with LEQEMBI administration. The blood biomarker results (plasma Aβ42/40 ratio, ptau181, GFAP and NfL) showed improvement even after delayed initiation of treatment with LEQEMBI.Since July 2020 the Phase 3 clinical study (AHEAD 3-45) for individuals with preclinical AD, meaning they are clinically normal and have intermediate or elevated levels of amyloid in their brains, is ongoing. Eisai also announced initiation of a Phase II study (Study 202) on E2814 for sporadic early Alzheimer’s Disease (AD).Impact of the anti-MTBR tau antibody E2814 on tau pathology biomarkers in Dominantly Inherited Alzheimer’s Disease (DIAD)E2814 is an investigational anti-MTBR tau antibody designed to target the MTBR of tau. In AD patients, neurofibrillary tangles (NFT) are a pathological hallmark, and they are believed to spread through synaptically connected pathways in the brain, forming the tau propagation hypothesis. It is thought that tau propagation is drivenby the specific tau species containing MTBR, tau seeds that spread tau pathology to different brain regions important for cognition and function.Eisai conducted a Phase I/II clinical study (Study 103, NCT04971733; 7 participants) of the anti-MTBR tau antibody E2814 in patients with Dominantly Inherited Alzheimer’s Disease (DIAD) beginning in June 2021. This study aimed to evaluate the safety and tolerability of E2814 in DIAD patients, with a primary objective of assessing the target engagement of E2814 with MTBR-tau species in their cerebrospinal fluid (CSF). In addition, pharmacodynamicevaluation was performed using multiple biomarkers related to AD tau pathology. In the study, DIAD patients withclinical symptoms were administered E2814 for 12 to 24 months. Data from the Dominantly Inherited Alzheimer Network Observational Study (DIAN-obs), an observational cohort of DIAD, were used as references to evaluate biomarkers changes in E2814 treatment.Compared to the reference data from DIAN-obs, patients who received E2814 showed approximately 75% and 50% reductions of CSF MTBR-tau243 and p-tau217, respectively, reflecting tau pathophysiology. Additionally, braintau accumulation observed by tau PET was stabilized or trended toward decrease in DIAD subjects administered E2814. These results suggest that E2814 inhibited tau propagation and suppressed the accumulation of tau aggregates in brains of people living with DIAD. This will be further investigated in the ongoing Phase II/III Tau NexGen study (NCT05269394) with DIAD patients and the Phase II 202 study (NCT06602258) with sporadic early Alzheimer’s disease (AD) patients.Initiation of Phase II clinical study (Study 202)In September 2024, Eisai initiated a Phase II clinical study (Study 202) for individuals with early AD in the United States. Democrats Show Improved Economic Standing With Voters

It happened without much notice, but Democrats have managed to close the gap with voters on who they trust to handle the economy after a decade of lagging behind. With just a week left before Election Day, Republicans’ advantage on this key issue has shrunk as Kamala Harris takes the lead. It’s important to note that Democrats haven’t completely overtaken Republicans on the economy. Republicans, led by former President Donald Trump, still hold an edge, largely because they are perceived as the party more supportive of free markets. However, over the past few years, the gap has been slowly narrowing, and this trend accelerated after Harris became the Democratic presidential nominee in July. Some Democratic moderates who favor Wall Street are quietly pleased, seeing this as a sign that the progressive wave that gained momentum during Bernie Sanders’ rise and the social changes of 2020 has subsided. The extent to which Democrats have made progress on this crucial issue varies depending on which polls you look at. The New York Times/Siena College poll last week showed Trump ahead on the economy with 52% to 45%, a margin that is outside the poll’s margin of error. However, Democrats point out that this is a tighter gap than the 54%-41% split found in the Times’ September survey. And it’s a significant improvement from May, when the same pollsters found Trump leading Joe Biden 58% to 36% on this issue. But Harris’ gains may be even more significant in the states where she needs them most. Shortly after Biden withdrew from the race, center-left pollsters Redfield and Wilton found Trump leading Harris on the economy in the seven key swing states, with Trump ahead by anywhere from six points in Michigan to 13 points in Pennsylvania. Now, however, these pollsters find Harris favored on the economy in three of those states and tied in another two. It’s worth noting that these advantages in the slightly Democratic-leaning polls range from 4 points up in Wisconsin, 1 point up in Georgia and Nevada, and tied in Michigan and North Carolina—all within the margin of error. But in states like Georgia, Nevada, and Pennsylvania, this represents a double-digit swing in her favor. It’s not a guaranteed win, but this shift has given some strategists on the left a sense of optimism that Democrats have at least closed the trust gap on the economy, which is expected to be the top issue for voters. “For years, Democrats have struggled to find a way to communicate their economic message even when the economy improved under Biden, especially against a businessman who is a television personality,” says one experienced strategist working with liberal groups this election cycle. “Democrats have been trying to figure this out for 10 years, and nothing has worked until now.” This optimism is not without merit. However, it’s important to acknowledge that this is not the most favorable position Democrats have found themselves in on the economy against Trump. When the Times asked voters about Trump and Biden’s handling of the economy in October 2020, they found the sentiment was essentially dead even at 47%-48% in Trump’s favor. Strategists from both parties argue that 2020 should be considered an outlier, a complete anomaly of a campaign season due to the ongoing pandemic. Assuming 2020 is unique, let’s look back at 2016 when Trump faced off against Hillary Clinton. In that race, Trump had a 10-point advantage over Clinton among voters who ranked the economy as their top issue. Harris is doing better than that, but not by much depending on the poll you’re looking at. While these shifts are small, it’s important to understand why there’s no celebration in Harris’ camp, especially considering that many voters seem unconcerned with the economic realities under the Biden-Harris administration. All signs point toward a strong economy in the foreseeable future. Unemployment is low, interest rates are expected to decline, and consumer spending is stable. Federal officials announced on Wednesday that gross domestic product, adjusted for inflation, grew at a solid 2.8 percent annual rate in the third quarter. Nevertheless, voters are feeling the pinch of inflation, and Trump has made the higher costs of everyday living one of the most persuasive and factually accurate points of his campaign rhetoric. Given this inflation-driven skepticism, it’s remarkable to see Democrats nearly reaching competitive parity on this issue, which far surpasses all other issues in importance; a Times poll found that you would have to combine abortion and immigration to reach the level of interest in the economy. Arguments for the shift toward Harris are varied. Biden’s departure ended the so-called “lesser of two evils” dynamic that alienated voters on the Democratic Party’s left flank, giving Harris some space free from criticism. Harris’ economic promises seem grounded in capitalism and less pandering than some of Biden’s efforts to appeal to the progressive movement. Harris represents a newer generation of politicians, someone who hasn’t been in the public eye to the point of saturation or bound to decades of promises. While Biden spoke in terms of fairness and equity, Harris is proposing concrete programs designed to make life easier for everyone at every stage. Among establishment-minded Republicans leaning toward Harris, a steady hand—even one that would raise taxes on the wealthy—is preferred to an erratic president who has proven inconsistent and unpredictable. Perhaps the biggest factor is that the frustration with Biden’s handling of the economy did not transfer to her; an NPR poll from August found that Trump still had the edge over Harris on the economy, 51%-48%, but this was a significant improvement compared to Biden’s deficit at 54%-45% before he dropped out. The answer is likely a combination of all these factors. If the numbers are to be believed, it seems Democrats have shed some of the baggage they carried for so long with voters. Why Howard University Holds Special Significance for Kamala Harris
After she received the call this summer that President Joe Biden had chosen her for the Democratic nomination, she immediately sprung into action, working the phones in her Howard University sweatshirt. The sweatshirt isn’t just a callback to the Vice President’s alma mater; it’s also a symbol of the historic nature of her candidacy. If Harris wins the election, she will be not only the first woman President, but also the first graduate of a historically Black college (HBCU) to hold the position. Harris matriculated at Howard in 1982, earning her bachelor’s degree in economics and political science. She has referred to her time at Howard as her “formative years” where she first found her love of politics. “For me, Howard is home,” Harris wrote in a January 2021 Instagram post, recalling her first experiences running for student office and campaigning. “Howard taught me that while you will often find that you’re the only one in the room who looks like you, or who has had the experiences you’ve had, you must remember: you are never alone.” Howard University is one of approximately 107 HBCUs across the U.S. While many HBCUs have been struggling, Harris has used her platform at the White House to support the institutions. Earlier this year, Harris invited Howard’s men’s basketball team to the White House, saying that investing in HBCUs is an investment “in the strength of our nation for years to come.” The support goes both ways: A “Bison for Kamala” PAC, referencing Howard’s mascot and organized by The Collective PAC, a group working to increase Black representation in politics, raised $250,000 in support of Harris’ candidacy as of October 20, according to reports. Harris has remained steadfast in her support for Howard over the years. In 2017, the year she was sworn in as a California Senator, she delivered the university’s commencement address. During her speech, she urged graduates to pave their own paths, reminding them that Howard “encourages us—expects us—to use our voice.” Harris has also referenced a number of Howard’s famous alumni who have inspired her— in her 2019 memoir, The Truths We Hold, she wrote that she decided to attend Howard because she wanted her career to “get off on the right foot. And what better place to do that than at Thurgood Marshall’s alma mater?” And, in 2019, she formally announced that she was running for President at Howard University. “This is where it all began,” she said. (She would go on to drop out of the 2020 Democratic primary before being chosen as now-President Joe Biden’s running mate.) These efforts culminated in a multi-volume study comprising drill logs, analyses, drill plans, maps and sections, deposit model studies, petrographic studies, metallurgical studies and more, culminating in a multi-volume compendium of reports produced by the Slovak Geological Institute published in 1992 (the "1992 Report"). Upon completion of the Amalgamation, the Company will move forward to verify this historical estimate by confirmation drilling so that it is able to classify mineral resources at Trojarova as current, in accordance with National Instrument 43-101. The nearby Medvedi-Potok property hosts a classic tin vein system in a greisened intrusive; it features underground workings and a historical (non-compliant) resource. Trump to Campaign in Wisconsin With Brett Favre, Amid Mississippi Welfare Scandal
Donald Trump and Kamala Harris will be campaigning in several swing states on Wednesday. They will cross paths in Wisconsin, where Trump will be joined by former NFL quarterback Brett Favre in Green Bay. Favre, a longtime supporter of Trump and other Republicans, spent most of his career playing for the Green Bay Packers. He won the NFL’s Most Valuable Player award three times and a Super Bowl. However, he has been in the news recently for a welfare scandal in his home state of Mississippi. While not facing criminal charges, Favre is among more than three dozen people or groups being sued by the state of Mississippi. The state is trying to recover misspent money. Favre has repaid over $1 million in speaking fees he received. These fees were funded by the Temporary Assistance for Needy Families Program (TANF). Mississippi Auditor Shad White, a Republican, has stated that Favre never actually delivered the speeches. White also mentioned that Favre owes nearly $730,000 in interest. Favre has posted and reposted social media messages supporting Trump and criticizing Harris, the Democratic presidential nominee. “In all the elections I’ve seen over my lifetime, I’ve never seen one where there was so much hate,” Favre posted on X, formerly Twitter, on Monday. “It’s certainly sad to see.” With six days left until Election Day, Harris will travel from Harrisburg, Pennsylvania, to Madison, Wisconsin, and then on to Raleigh, North Carolina. Trump will be traveling in the opposite direction, heading to Green Bay after an event in Rocky Mount, North Carolina. The focus on Wisconsin is not surprising. Wisconsin is a key battleground state in presidential elections, often decided by a very small margin. Trump won the state in 2016 by 23,000 votes but lost it to Democrat Joe Biden in 2020 by 20,000 votes. By campaigning with Favre, Trump is aiming to leverage the state’s strong support for the Packers and their former star quarterback. However, Favre’s involvement also brings baggage due to the Mississippi welfare spending scandal. Mississippi has been ranked among the poorest states for decades, and the TANF program has been designed to help families in need. However, the Mississippi Department of Human Services allowed well-connected individuals to misspend tens of millions of welfare dollars between 2016 and 2019. This was revealed by White and state and federal prosecutors. A nonprofit group called the Mississippi Community Education Center made two payments of welfare money to Favre Enterprises, Favre’s business: $500,000 in December 2017 and $600,000 in June 2018. The TANF money was supposed to be used for a new volleyball facility at the University of Southern Mississippi. Favre agreed to lead fundraising efforts for the facility at his alma mater, where his daughter started playing on the volleyball team in 2017. The director of the Mississippi Community Education Center, Nancy New, pleaded guilty in April 2022 to charges of misusing public funds. Her son Zachary New, who also worked with the nonprofit, pleaded guilty to the same charges. They are both awaiting sentencing and have agreed to testify against others. Favre testified before a Republican-led congressional committee that was investigating how states are struggling to effectively use welfare programs to help families in need. Some members of the committee have said the Mississippi welfare misspending scandal involving Favre and others highlights the need for “serious reform” in the TANF program. Favre told the congressional committee that he was unaware of any wrongdoing in January.
