New Drug Shows Promise in Improving Transplant Success

Organ and cell transplants are potentially life-saving treatments for a variety of medical conditions, but their success depends on how long the transplanted cells or tissues can survive in their new host. Drugs that suppress the immune system to minimize rejection must strike a delicate balance between being powerful enough to hold back the recipient’s immune system from destroying the donated material, without being too potent to cause toxic reactions. In a study presented at the in Boston, researchers report on encouraging results using an experimental immunosuppressive drug in people with Type 1 diabetes. People with the disease no longer make enough insulin in their pancreas to break down sugar in the foods they eat, and have to monitor their blood sugar levels and inject themselves with insulin for the rest of their lives. In some cases, the insulin shots become ineffective and patients need more aggressive treatment: usually a transplant of islet cells—which produce insulin in the pancreas—from a deceased donor. But many patients end up rejecting the transplanted cells when their body’s immune system views them as foreign and starts attacking them. In the latest study, researchers led by Dr. Piotr Witkowsky, professor of surgery and director of pancreas and pancreatic islet transplantation at University of Chicago Medicine, performed islet cell transplants using a different drug to suppress the recipients’ immune systems. The drug, tegoprubart, was the same one used in the first earlier in 2024. In the current study, three patients received transplants of islet cells with tegoprubart; two no longer needed insulin injections after 18 weeks (and after a second islet cell transplant) and four weeks, respectively, and achieved normal A1C levels, while the third patient reduced their insulin needs by 60% several days after the transplant and is still being monitored. “We have been doing deceased donor islet cell transplants for the last 24 years,” says Witkowski. “There was a lot of hope at the beginning that patients might come off insulin, but we realized that the immune suppressants we were using were not optimal, and over time, patients were losing the [transplanted] islet cells.” The small number of patients in the tegoprubart study reported no side effects, and the transplanted islet cells were three to five times more likely to engraft and produce insulin than cells transplanted in people who had received the current immunosuppressive drug tacrolimus. “We don’t need to adjust doses to control toxicity like we do with tacrolimus,” says Witkowsky. “And their islet function is at least three times better compared to patients receiving tacrolimus because there is no toxicity. These results are preliminary, but the hope is great.” Witkowsky was inspired to try tegoprubart in islet cell transplants after the success among kidney transplant patients—including the first pig kidney operation and more traditional kidney transplants using donated human organs. Studies showed that among 60 kidney transplant patients receiving tegoprubart, there were no rejection episodes and no toxicity. “The kidney function in those transplant patients seemed to be better with tegoprubart compared with tacrolimus, and we think we can get similar results with Type 1 diabetes,” he says. The new drug is an antibody, given as a 15-minute infusion once every three weeks. It works by suppressing the immune response to foreign proteins in a transplant from a donor. It’s not yet approved; the company developing it, Eledon, continues to study it in clinical trials to assess its safety and efficacy in kidney transplants, animal organ transplants, and ALS. Earlier versions of the drug studied more than two decades ago increased the risk of blood clots, but continued research has reduced that risk and improved tegoprubart’s immune-suppressing ability. Witkowsky hopes to find funding to continue studying these first patients and add six or so more to better understand how long the islet cells can survive and whether the drug can buy the transplanted cells enough time to engraft and restore patients’ insulin-making functions to be as close to normal as possible. “Unfortunately there is no [real] therapy for type 1 diabetes patients,” he says. “The bottom line is that we know the cells have the potential to work—they do work. The problem remains immunosuppression. And now we have a medication that may help us a lot.”

Trump Calls New York Rally Filled With Insults a ‘Lovefest’

(PALM BEACH, Fla.) — On Tuesday, Donald Trump referred to his rally at New York’s Madison Square Garden, an event featuring numerous speakers, as a “lovefest.” This is the same term the former president has previously used to describe the January 6, 2021, riot at the U.S. Capitol. Speaking to reporters and supporters at his Mar-a-Lago resort, Trump asserted that “there’s never been an event so beautiful” as his Sunday night rally in his hometown of New York City. “The love in that room. It was breathtaking,” he stated. “It was like a lovefest, an absolute lovefest. And it was my honor to be involved.” This comes despite criticism from Democratic Vice President Kamala Harris’ campaign and many observers, including Republicans, concerning racist remarks made by speakers during the pre-show targeting Latinos, Black people, Jews, and Palestinians, alongside sexist insults directed at Harris and former Secretary of State Hillary Clinton. Comedian Tony Hinchcliffe’s set, in which he joked that was a “floating island of garbage,” sparked particular outrage given the electoral significance of Puerto Ricans living in Pennsylvania and other key swing states. The Trump campaign took the unusual step of distancing itself from Hinchcliffe’s joke about Puerto Rico but not other comments. Deborah Lipstadt, the U.S. Envoy to Monitor and Combat Antisemitism, brings extensive experience and credentials to her role. Following the outbreak of the Israel-Hamas war on Oct. 7, 2023, Lipstadt spoke with TIME about the evolving nature of her job and her perspective on the global response to the conflict. This interview has been edited for length and clarity. You are the President’s special envoy to monitor and combat antisemitism. How’s it going? I'm quite busy, and I actually hope for a decline in the amount of antisemitism I see. Does that mean your job has gotten easier or harder in the past year? When I first took office, I emphasized the need for people to take anti­semitism seriously. The notion that “Jews have it made” was a common sentiment I encountered. Now, I hear more from people who are concerned about their safety, like advising their 12-year-old grandson wearing a kippah to wear a baseball cap for protection, even in places like the Upper East Side of Manhattan. You grew up when Israel was the underdog. Whole generations have known it first as an occupier. I was in Israel during the Six-Day War. I was a child then, and we were uncertain about the future. Israel's public image has changed dramatically since then. However, there remains a significant amount of hatred directed at Israel from various entities who wish to see its downfall. How can one distinguish between criticizing Israel and being antisemitic? Holding all Jews responsible for actions taken by the Israeli government is antisemitism. But if criticizing Israel's policies constituted antisemitism, then the hundreds of thousands of Israelis protesting in the streets on Saturday nights would be antisemites. Your academic work is centered on the Holocaust. Is hearing what’s happening in Gaza described as a genocide triggering in any way? There is a specific definition of genocide. We can acknowledge the tragedy unfolding in Gaza, recognizing that many residents are not supporters of Hamas. The suffering is immense and seemingly unending, but it doesn't constitute a genocide. Between what happened in Israel on Oct. 7 and in Gaza afterward, sometimes it can seem like the traumas are in competition. There are certainly competing traumas. However, I don't engage in comparing suffering. Having two impacted molars doesn't make your one molar feel better. I don't think it's productive. We are talking about responding to an attack. The 1,200 deaths on Oct. 7 are equivalent to 48,000 American deaths in proportion to our population. If anyone had suggested we remain silent after 9/11 and not retaliate, it would have been unthinkable. If someone attacks, you have to respond. Did you just say we? That's right. You raise a good point. I was speaking as an envoy for Joe Biden, who flew to Israel after the attack, and also as a Jew. Do you think Jewish people in general feel as if their fate is attached to Israel’s? Some Jews do. Some Jews believe that if anything were to happen to Israel, they would be less safe in the world. Many Jews feel this way. Does it work the other way? If ­Israel is delegitimized—a big word inside Israel—are Jews more vulnerable? I think so, in many places. We must also consider the situation with the Uighurs. You want to talk about a genocide? Talk about the genocide of the Uighurs. That’s not happening on camera though, is it? The Chinese government has ensured that. But if someone were to find a group of Chinese nationals and beat them up [in retaliation], we would be appalled. Rachel Weisz played you in Denial, the movie about your being sued for libel by a Holocaust denier. Are you still in touch? We email each other. After I was appointed envoy, she told the producers they needed to call her "ambassador." She took the role very seriously. Her father escaped Hungary, and her mother was born in Vienna to a Jewish father, and they had to flee. So she approached the role personally.

Restoring Nature Is Essential for a Sustainable Future

“As the planet heats up, we are facing growing problems of either too much or too little water.” This was the warning from Celeste Saulo, Secretary General of the World Meteorological Organization, this month. This work cannot be done by individual nations alone; it requires a coordinated global effort to meet these existential threats.   The root cause of nature and biodiversity loss lies in how we use land. And agriculture is one of the main drivers. Over 75% of ice-free land on earth has been altered by human activity, primarily for food production. As we continue to convert wildlands to agriculture, we degrade ecosystems, pushing countless species to the brink of extinction and devastating livelihoods for local and Indigenous communities. Unsustainable agricultural practices further exacerbate biodiversity loss and the decline of ecosystems that provide the basics for sustaining life like clean air and water, and fertile soils. But we can produce healthy and nutritious food in a sustainable way by effectively managing and restoring our productive lands. U.S. Secretary of State Antony Blinken has put it quite simply, saying in a speech earlier this year, “without good soil, crops fail, prices rise, people go hungry.” There are important global meetings this fall—known as Conferences of the Parties (COP)—which will provide global leaders critical opportunities to unite around the important cause of bringing our earth’s systems back into balance—and find the funding to do it. In November, the COP28 will focus on financing for climate mitigation and adaptation. Meanwhile, the COP16 currently underway in Colombia centers on halting the loss of nature and finding financing to achieve the ambitious global goal of conserving 30% of the planet by 2030.   A third key event, the COP16 of the UNCCD, will spotlight the restoration of degraded lands and the need of building drought resilience working with communities, companies and citizens in rural and urban landscapes around the globe. The UNCCD is a critical global voice for conserving and restoring land, and one of the three major U.N. treaties known as the Rio Conventions, alongside climate and biodiversity. The Kingdom of Saudi Arabia is leading the way to make this UNCCD COP16 in Riyadh “a moonshot moment to raise global ambition and accelerate action on land and drought resilience through a people-centered approach.”  The benefits of restoring degraded lands are vast. Roughly 40% of economic output—more than half of global GDP—depends on functioning natural systems. Restoring land is not just an environmental issue; it is key to addressing global challenges such as food and water insecurity, human migration, and poverty. As UNCCD Executive Secretary Ibrahim Thiaw has said: “By restoring land, we restore life, restore our economies, restore our communities, and so much more.
